Here's a list of books I own on revival that I'll be using in one way or another for my current sermon series Revival: Longing for a Surprising Work of God.
I tried to list them in the order of how much I expect to use (or depend on) each one for this series. I'm referencing a number of other things as well (articles, audio, websites) but this list is for books alone.

Too many good books on revival are out of print, such as Edwards.

Steve, Great list! Did you happen to take Nettles' Revivals and Revivalism class at SBTS? It was fantastic; we read a lot of the stuff on your list. I've found When God Comes to Church, by Ray Ortlund to be fantastic as well.

John Kitchen wrote a book called "Revival in the Rubble." It is not the most popular of books but helpful nonetheless. He walks through the revival that took place in Nehemiah's day. Also, you might want to check out www.lifeaction.org.

As well as Brian Edwards book 'Revival' he has written a second one 'Can we pray for revival?'

I'm a little late to the party here, but I would add to the mix a book by John Thornbury - God Sent Revival: The Story of Asahel Nettleton and the Second Great Awakening. It's essentially a bio. of Nettleton.

Steve, I'd encourage you to check out Ray Ortlund's book "When God Comes to Church." It's out of print but you can find it used online. Great sermons on revival from the Old Testament.

Hey Joshua. I actually talked to Ray about whether the book coming back in print. It isn't, and it's not cheap used. I was hoping to find it at a used store a couple of weeks ago, but didn't. Didn't know if it was worth $35. Thanks for the suggestion. I'll keep looking.
